The following links allow you to download printable versions of our prescription forms. Simply print them out, fill out your prescription, keep a copy for your records and include a copy with your case and ship to DDS Lab.
To fill out your prescription online:
- Go to My DDS Lab and enter your user name and password and log in.
- Click on "Add Case" and enter your prescription details in the form fields.
- When prescription is complete, print two copies, one to keep and one to send with your case.
- Next, pack your case, apply our preprinted FedEx Express Billable Stamp (shipping label) to the outside of the box and dial 1-800-GoFedEx to schedule a pick up.
